The required Patient In-take Forms are provided below to help save you time during your first visit to HealthCARE Express.
While HealthCARE Express provides services on a walk-in basis, ZipPASS is designed to provide greater convenience for you. ZipPASS puts you on our schedule from the comfort of your home or office. Your ESTIMATED time is based on patients at our clinic now & could change upon arrival. Please come in at your estimated appointment time.
ZipPASS Appointments are only available weekdays, 2 hours after the clinic opens until 2 hours before the clinic closes. Please note you may still come in without an appointment to be seen.
First time patients can download the patient registration form by clicking the link above. Complete the indicated fields and present the completed form to our receptionist upon arrival. If you have questions about your registration form, our receptionist will be able to assist you upon arrival.
HealthCARE Express accepts most health care insurance plans serving the majority of customers in each market. We are continually being added as a provider to group health plans, so be sure to call our office or check back here often for the latest list of insurance we accept.
Please keep in mind, insurance companies want to keep you out of the emergency room! We have found that many insurance providers cooperate with urgent care centers even if they are not formally contracted.
If we do not currently participate with your insurance provider, you would initially need to pay as if you were a self-paying patient. However, we recommend contacting your carrier to verify coverage policies with out-of-network urgent care providers. With a commitment to provide our patients with the highest level of customer service, HealthCARE Express will assist you in submitting any medical claims to your insurance company during check-out.

HealthCARE Express understands that the cost of care is often more important to our customers than convenience. For patients and families who plan to pay without insurance, we provide a discounted fee schedule that is among the most competitive and affordable in our market.
Payment is required at the time that services are rendered unless you are a member of a participating insurance plan of HealthCARE Express. Any co-payment and deductible will be collected at time of service before you are seen. The cost of any additional services rendered for your treatment shall be made upon check-out. Payment can be made by cash, personal check or credit card. It is company policy to run your check by EFT or your credit card. For private pays (no insurance) all charges for the visit is due before you are seen. Please note that you may have a balance at the end of your visit, which must be paid before you exit the office.
